CARE PATHWAY

1 Create an access profile

for your patient, select a follow-up protocol that you have defined for the procedure and which is fully configurable to account for each individual’s characteristics.

2 Before the operation

the patient completes his/her check-list and accesses the explanatory content relating to his/her condition and procedure. The patient may, if he/she wishes, share the content with family members so as to be well prepared on the big day.

3 After the operation

the first Maela postoperative questionnaires can be directly input from the healthcare facility. The medical staff confirm with the patient whether he/she is ready to continue the follow-up from home.

4 The patient follows the follow-up protocol at home

defined by his/her practitioner. Questionnaires, treatment reminders, measurements or photos, everything that a nurse would have checked at hospital can be performed directly on the Maela platform. The patient also has round-the-clock access to our nurses to ask them for advice.

5 A nurse monitors the data 24/7

responds to the patient’s queries and concerns throughout the follow-up period and acts as an interface with the practitioner in the event that the reference values defined for follow-up are exceeded.
